How Self-Referral Works

Self-referred patients begin their visit through our General Dentistry Service consultation pathway.
After reviewing your submission, our team will contact you to schedule a consultation where we will:

Evaluate your pet’s health & determine if pre-anesthetic diagnostic tests are necessary

Discuss dental findings and treatment options

Determine the most appropriate care plan
If specialty-level dental care is required, treatment may scheduled with our Specialty Dentistry Service.
Submitting this form does not schedule treatment. It allows our team to review your pet’s information and arrange the appropriate consultation.
Before you Submit
If you are hoping for your pet to be considered for treatment with our Specialty Dentistry Service, your pet must first be examined by their primary veterinarian.
